Radiology: before and after

This teenager has Sotos syndrome, a congenital condition. As part of the spectrum, he had this very substantial scoliosis. The X-ray appearance suggests that he would have very considerable symptoms, but in fact, he managed fairly well.


Surgical correction was undertaken in a two-stage procedure, requiring some very substantial metalwork to fuse and hold his spine in a near normal position. He has done very well following the operation.

This elderly male patient had a history of rickets, a previous fractured neck of femur and a total knee replacement. He slipped while at the supermarket, producing a stress fracture in the femoral shaft just below the DHS plate.

The repair involved the positioning of a new long plate, fixing the femur while retaining the integrity of the hip repair. Sadly, after making a great recovery, the patient fell at home and sustained a major injury to his neck, leaving him paraplegic.

This man sustained a serious fracture of his pelvis in a skiing accident. The posterior wall of the acetabulum was the site of the main injury, rather than the femur, which normally breaks. He was kept on traction for a couple of days before surgery was undertaken.

The repair was relatively simple, with screws holding the pelvic fracture in place, returning the ball and socket to normal. There was no obvious nerve damage and he has had no significant problems since the operation. He is now back in the gym on a regular basis.

This 18-year-old man fell on his elbow, sustaining this comminuted fracture of the distal humerus.

Surgery was required to try to reduce and fix the fracture and restore the anatomy, allowing the bones to heal and minimising the patient's short-term problems, as well as the risk of developing osteoarthritis in the future.
