Maternity inquiry finds lack of systematic approach to safety creates risks

The lack of a systematic approach to ensuring safety across maternity services is creating unnecessary risks, according to a King's Fund inquiry.

It concluded that the overwhelming majority of births in England are safe despite growing pressures on maternity services but called for a range of practical improvements.

Recommendations to improve safety included better team and multidisciplinary working, more safety training and good management of staffing levels and skill mix.
