With a zero tolerance policy patients can be removed immediately without writing to the patient. If a member of staff or doctor is at risk of physical harm, you inform the police and obtain the incident number, then write to or email the primary care organisation, which has a special allocation scheme under which this patient can be treated.
I am not sure whether this is only in south-east Essex, but it seems to work both for patients and the practice.
