The Capability Report commissioned by Cabinet secretary Sir Gus O'Donnell outlines a raft of key failings and shortfalls.
The report is critical of the volume of initiatives introduced that have not been properly thought through, and says the DoH is failing to learn from its mistakes. Relations with front-line staff are critically strained.
The authors identify 'serious concerns' in leadership and find morale is in urgent need of improvement in a DoH, lacking a 'clearly articulated vision for the future of health'.
But 'encouraging progress' has been made in key areas such as cancer inequalities, heart disease, 18-week waits, patient experience and reducing smoking.
The Home Office was rated the worst department.
