DoH launches long-term conditions guide for GPs

The DoH has launched an information booklet and online resource pack to help GPs in the management of long-term conditions.

The booklet, entitled Your health, your way, is designed to help GPs promote the idea of self care to their patients.

The booklet contains information on the importance and value of self care for those with long-term conditions, as well as advice on healthy living and tools for monitoring patients with long-term conditions.

Dr David Colin-Thome, director for primary care in England, said: ‘While GPs are already supporting people to self care, a resource which can help busy GPs to engage patients in self care is very welcome as evidence confirms it benefits the patient by improving health related quality of life.’
