RCGP Scotland chair Dr John Gillies said: 'We want to hear from patients in their own words so that we can share good practice, define what makes the best GP and ultimately improve quality of care. It is vital that we recognise what is working well in our practices and communities and make sure that we use it in future.'

The patient who submits the winning entry will be invited to present the prize to their GP at the awards evening on 11 November 2011 in Edimburgh, which will be attended by the leaders of primary care, MSPs, patients and practice teams.

Also presented at the event will be The Practice Team Award which recognises the dedication of all members of the healthcare team, from administrative staff to practice nurses.

Alongside the Practice Team Award will be the award for service to general practice, particularly in a local community setting, known as the Alastair Donald Award. The closing date for entries is 8 August 2011.
