Previous winners

Overall Winner 2011

South Holderness Medical Practice, East Yorkshire

This practice showed exemplary teamwork in its one-stop approach to reviewing patients with more than one long-term condition. The semi-rural area has high levels of deprivation, with prevalence of conditions such as diabetes running at 6.7%. The practice launched an innovative approach to the problem, bringing together GPs, nurses, pharmacists and patients, to improve the quality and efficiency of care. In the next year they completed combined health checks on more than 1,000 patients.

Overall Winner 2010

Upton Surgery, Worcestershire

This practice’s ‘complex care team’ provided a multi-agency approach to supporting patients with acute needs that saved £148,000 in one year. Other practice activities have included walks and exercise classes, on-site mental health services, expert patient programmes, a toe-nail cutting service and a pulmonary rehabilitation course. They have also brought acute services closer to home, with consultant clinics in rheumatology, older age medicine and paediatrics, and specialist nursing in audiology, mental health and heart failure.

Overall Winner 2009

SSP Health, Wigan, Lancashire

This surgery was seeking a simple solution to its access problems – surgeries were fully booked but there were DNA rates of up to 20%. They bought a mobile phone and asked patients who could not attend their appointment to send a text message including the word ‘cancel’, their name and the date of the appointment. The dedicated phone is kept at reception so that staff can action the cancellations as the texts arrive.
