Practices to prepare care plans for patients with long-term conditions

Guidance on planning personalised care for people with long-term conditions has been issued by the DoH.

Care planning should mainly take place in primary and community care, the DoH says. It will involve staff GPs, practice nurses, community matrons, allied health professionals and social care workers.
 
The recommendations for commissioners outline how planning personalised care can have benefits for individuals, providers of services, commissioners and the health and social care workforce.

Benefits include efficiency savings such as reductions in GP consultations, as well as  hospital admissions and outpatient appointments, the guidance document says.
